My technique is to use a shutter speed that is only just long enough to blur the background - any longer and you risk making the whole shot blurry. Then follow the subject tapping a half press every so often to update the focus (on an SLR anyway), when you are ready to take the shot, half press then full press whilst continuing to pan with the subject (most important!!!). The trickiest bit I find is keeping a smooth pan while pressing the shutter.
